Advance Directives

Advance directive is a set of written instructions specifying what actions you would want taken for your health if you should become unable to make decisions due to illness or other incapacitating factors. You have the right to provide an advance directive to University Health Services (UHS) and have UHS providers and staff comply with your instructions.

Find forms related to advance directives. For help developing or completing an advance directive, go to Legal Services for Students and complete an online “Request a Consultation” form or call (512) 471-7796.

Take completed advance directives to the UHS General Medicine check-in desk (SSB second floor). Staff will route it to be incorporated into your health record.
