Diet analysis form instructions:
- Print and fill out the food record Form. Write down everything you eat and drink for three days. For best results, record what you eat as you go, not at the end of the day. Try to be as accurate as possible, and use descriptive and objective measurements such as "tablespoon", "piece the size of a fist", or "ounce", instead of subjective measurements like "some" or "a bowlful."
- Bring your completed form to us (SSB 1.106). We will e-mail or call you when the results are ready to pick up.
