Your feedback can make a big difference. Because of patient input, we’ve added a wider variety of healthy snacks in the UHS Pharmacy and remodeled our nurse advice area to provide more privacy.

Providing the quality of our service is every UHS staff member’s goal, and your opinions are important to us. To that end, we regularly solicit feedback about our services through Patient Satisfaction Surveys and Student Feedback Forms.

Patient Satisfaction Surveys

We email web-based Patient Satisfaction Surveys to 10% of the patients seen in our various clinics and all of our Physical Therapy patients each day, asking them to "grade" us on a variety of measures. If you receive a survey from us, please complete it. Identifiers are stripped from your responses, so your feedback remains anonymous. Student Feedback Form

You don’t have to wait to get a survey to tell us what’s on your mind. If you have a compliment, suggestion, or concern, we'd love to hear from you!

It's easy:

  • Complete and submit an online Student Feedback Form (see Related Links)
  • Print and fill out a Student Feedback Form (see Related Links):
    • Drop it in one of the designated boxes in each UHS waiting area
    • Mail it to:

You can also pick up and drop off a Feedback Form in any UHS department waiting area. You may choose to complete the form anonymously, but if you want us to contact you to respond to your feedback, please include your name and EID. Any specific health information you provide is subject to strict confidentiality policies and will not be available for public review.
