Important Notice Regarding Social Security Numbers (SSNs)
Due to archiving limitations, UHS medical records generated for students before January 2005 are likely to include an SSN. Therefore, if you authorize release of UHS medical records generated prior to this date to any entity, your SSN will be disclosed to them. No law or authority requires you to disclose your SSN, but if you choose to withhold it, we will be unable to release your medical record to anyone other than you. If you do not want to release your SSN in your medical records, you will need to redact this information yourself.
To Request Release of Your Medical or Immunization Record
You can submit a request to have your medical or immunization records released to you or to another person or entity. To do so, complete an Authorization for Release of Medical Records and an Authorization to Disclose Social Security Number Contained in Medical Records (see Related Links). Please note that UHS retains medical and immunization records for seven years after the patient's last visit. Information on requesting release of Prescription Records can be found in Related Links.
There is no charge to get a copy of your immunization records.
Submit your authorization forms in person. Bring valid photo identification and pay for the release of your records (see fee schedule below). Your records will be mailed or ready for pick up within 5 to 15 business days. If you only need a copy of your immunization record, you can pick it up in person at no charge.
