Protect Texas Together Activity Pass
The Activity Pass is a feature of the Protect Texas Together app that displays a “cleared” status to participate in on-campus and in-person events based on COVID-19 test results. To enable this feature, first add your Activity Pass consent in the Protect Texas Together app. Clearance can be obtained via “not detected” results at a UT Proactive Community Testing (PCT) site.
Adding Activity Pass consent in the Protect Texas Together app
- Select Activity Pass on the app homepage.
- Select Add Activity Pass consent.
- Read the consent form and select Sign Consent if you agree.
- Sign the form and select Submit Signature.
Submitting Student Self-Declaration of Release from COVID-19 Isolation
Only complete this form if you meet all CDC criteria for discontinuing self-isolation:
- 10+ days since symptom onset (if no symptoms, 10+ days since test date) AND
- 24+ hours fever-free without use of fever-reducing medications AND
- Other COVID-19 symptoms have improved
- Log into myUHS portal
- Click “Messages” on the left sidebar.
- Click “New Message.”
- Select “I want to fill out and submit a Self-Declaration of Release from COVID-19 Isolation to UHS.”
- Complete the form.
- Submit the secure message.
