Self-reporting form for COVID-19

UT students: Please refer to the instructions below to report a positive COVID-19 test performed at any facility that is not University Health Services or UT Health Austin. Your self-report will initiate contact tracing, which helps slow the spread of COVID-19 by identifying close contacts of individuals who test positive and recommending they self-quarantine and get tested. Privacy and confidentiality will be maintained.

Individuals who test positive for COVID-19 and complete a self-isolation period may request a Protect Texas Together Activity Pass for residence halls, Athletics Big Ticket claims, or other campus activities. Visit the Activity Pass page for more information.

Faculty and staff who have symptoms or are being tested for COVID-19 should report their symptoms and/or test result to the Occupational Health Program.

Submitting COVID-19 self-report form

  1. Log into myUHS portal
  2. Click “Messages” on the left sidebar.
  3. Click “New Message.”
  4. Select “I want to self-report my positive COVID-19 test from a non-UT site (i.e., a site other than UHS, UT PCT, or UT Health Austin)."
  5. Complete the form, providing as much detail as possible.
    Note: If you are requesting an Activity Pass, uploading documentation of your test results is required.
    • Upload an attachment (PDF, JPG, PNG) documenting results.
    • Results must contain your name, test date, test type, and a positive result to be reviewed for Activity Pass clearance.
    • Results of serology (blood) tests for COVID-19 antibodies will not be accepted.
  6. Submit the secure message.
