How to Schedule a Flu Shot Appointment Online

If you have not read about the type of vaccine being administered, charges and payment methods, or the safety precautions being taken to keep you safe at UHS flu shot clinics, click here.

Read all information below to help your scheduling and check-in processes go smoothly.

You must do the following to check-in on the day of your flu shot appointment:


  • Complete the Symptom Survey feature in the Protect Texas Together app or, if you do not have a smart phone (e.g. iPhone, Android) The Protect Texas Together phone survey. Find details here. You must present your “Clear to Come to Campus” pass (app) or your Clear to Come to Campus email (phone survey).*
  • Wear a face mask and respect social distancing.
  • Unless you require assistance, do not bring anyone with you (including children)
  • Present your UT ID card.
  • Show the QR code you will obtain during scheduling (either as a screen shot, printed out, or by locating it in MyUHS under Appointments).*

* Alternative symptom checking and check-in mechanisms will be available at flu shot clinics for those without smart phones.

Instructions for Scheduling an Appointment

  1. Log into the MyUHS web portal
  2. If you are faculty/staff and get a message that you are not eligible to schedule, call 512/471-4955 to have this corrected.
  3. Click Profile to enter/update your contact information.
  4. Complete required UHS Consent Form and Health History Form, if prompted to do so. Click Health History Form on the left. To bypass completing the entire Health History, just enter accurate medications and allergies or, if true, click “No known allergies” and “No current medications.” Click “Done” when finished.
  5. Click Appointments on the left, then Schedule an Appointment.
  6. Select your Status. If you are a student who is also employed at UT, select “Student.”
    • If you clicked Student:
      • Enter your insurance information. If you have insurance and do not enter it at the time you schedule, you will be billed for the cost of the flu shot. If you HAVE entered your insurance information previously, click “Proceed to booking the appointment.” If you HAVE NOT entered your insurance previously, read “How to Enter Your Insurance Information” in the box below.
      • Click “I want to book my flu vaccine.”
    • If you clicked Staff or Faculty:
      • Select “I want to schedule my flu vaccine.”
      • Enter your insurance information. If you have insurance and do not enter it at the time you schedule, you will be billed for the cost of the flu shot. If you HAVE entered your insurance information previously, click “Proceed to booking the appointment.” If you HAVE NOT entered your insurance previously, read “How to Enter Your Insurance Information” in the box below.
  7. Follow prompts to schedule the appointment, selecting your preferred location at The Texas Union Ballroom.
  8. Save a screenshot of your appointment QR code, print out the QR code or access it later via the MyUHS Portal on your portable device when you check-in at your appointment.

How to Enter Your Insurance Information


  • Click “Messages” and then “New Message”
  • Select “I want to submit my insurance information to UHS and click “Continue”
  • Fill in the form. If possible, take a picture of the front and back of your insurance card and add it as an attachment to your message.
  • Begin scheduling process again.
