Before Your Appointment – Telehealth or In-Person

Complete the Pre-visit Questionnaire in the MyUHS/CMHC portal to let us know the reason for your visit.
Within the MyUHS/CMHC portal, complete these required forms and enter your insurance information, if insured, even if UHS is out-of-network with your plan.

a. Consent to Treat Form
b. Treatment Agreement
c. Notice of Privacy Practices
d. Charges Policy
e. Health History Form
f. Insurance Status Form

Review and make any necessary changes in your patient profile within the MyUHS/CMHC portal.

Ensure your name, preferred name, phone numbers, local address, email, preferred method of communication, pronouns and gender marker are current.

The MyUHS/CMHC portal allows you to update your name and pronouns in your medical chart.

On the Day of Your Telehealth Appointment

Check in online 15 minutes before your visit. The QR code sent to you within the MyUHS/CMHC portal is used for check in for in-person visits, not telehealth visits.

Locate your appointment in your MyUHS/CMHC portal, then select check in.

Enter your location if prompted.

Select Join Meeting, which will initiate your telehealth connection.

If you are more than ten minutes past your check-in time, you will not be able to check in and will need to reschedule your visit.

On the Day of Your In-Person Appointment

Please arrive 15 minutes early to maximize the interaction with your healthcare provider and avoid increasing wait times and disrupting schedules of students with appointments after yours.
